It
is normal for patients who may require orthodontic treatment
to see their dentist first who will then refer them to
the specialist orthodontist for a consultation that may
lead to a course of treatment. If, however, you are
not currently registered with a dentist then it is possible
to refer yourself for a consultation by completing the
form and e-mailing it to the Orthodontic Practice.
Complete
the box below and e-mail to the orthodontic practice
where the receptionist will contact you to arrange a
consultation with the orthodontist which will be free
to those under the age of 18. It is recommended that
you read the self diagnostic notes
describing the sort of conditions that would respond
to treatment because minor orthodontic conditions are
unlikely to be funded by the NHS.
